Sounds like there is a problem with the passenger boat certification process in South Africa.  When you add up 19 people plus equipment plus a big heavy stainless steel cage on a relatively small 11 meter boat, it's impossible to figure out how they did the math.  The VCG (vertical center of gravity) would have been unacceptably high. Top-heavy boats kill people, in this case, three.
