It is my understanding that Shari Booth died while participating in a PADI Advanced Open Water scuba diving course.  If that is correct, the dive instructor is 100% responsible for her death and should be sued for gross negligence. When I conduct Open Water and Advanced Open Water diving courses, I always use at least one experienced divemaster or another instructor for the open water training dives because it is imperative to maintain visual contact with all student divers at all times.
