Scuba diver died in the Philippines — what are the details?

Very sad to hear anything like this... we are all so keen to look after each other in the water...and even above.  What is not clear is how did this Japanese diver get separated from the team and end up at 90 meters? Does this have anything to do with a strong downward current? Or, was it his fault, or why? What are the details of this case? Does anybody know the facts?
