Didn't Peter Hughes hire an inexperienced boat captain and promote him?...

IMMARBE's report points the finger at the young, inexperienced boat captain but Peter Hughes hired him and promoted him. Also why didn't Hughes monitor the hurricane warnings and work closely with his employees including the Wave Dancer captain to protect the passengers?  That doesn't really add up, does it?  And what about the Hughes hurricane response plan that did not stipulate mandatory evacuation.  I guess that's now required but sadly, it's too late for the Wave Dancer dead.
