According to Ian H "PADI's diver training system teaches us that every dive and every diver's responsibility for the safety of the dive starts with a critical assessment of conditions before donning one's scuba diving equipment".

Could you please explain why a PADI instructor drowned whilst retrieving a snorkel from the pool?

PADI instructor dies after freak pool accident

Evidently PADI fail to teaches divers that a safe dive starts with making it out of the pool first.
