You're right Lucas: BSAC and PADI both lack integrity...

Thanks Lucas.  The "accident" and "incident" definitions you posted from the compact Oxford dictionary confirm that BSAC is in error.  The correct word for an annual report on scuba diving accidents is "accidents", not "incidents".

Of course, if we wanted to downplay the inherent risks of scuba diving, as BSAC corporate execs obviously do, we could replace "accidents" with the circumlocutionary "unfortunate incidents" as in "BSAC Diving Unfortunate Incidents Report".  But not only is that a mouthful, it's a mouthful of BS (or should we say a mouthful of BSAC?).

As for your "bait and switch" to PADI's flagrant abuse of the language, I'm not biting.  PADI's failings have been well-documented and thoroughly criticised at Scuba Forum and elsewhere. What good does it do to point out that BSAC and PADI are on the same (dirty) page?  Isn't that a step backwards if you count yourself among Brits and anglophiles who consider BSAC superior because 1) it's British, 2) it's a club and 3) (my personal favorite) the water is different in the UK?
