The dive industry does not adhere to its own safety recommendations...

The dive industry--meaning dive centers and live-aboard operators like Mike Ball, Peter Hughes and the Aggressor Fleet--do not adhere to industry safety recommendations.  They are not required to and for the most part, they don't.  That's one of the points Mr. Suzuki make in the CDNN editorial:

"PADI recommends that divers who complete the course restrict their depth to 12m/40ft. PADI also recommends that Open Water divers restrict their depth to 18m/60ft. Yet almost everyone, divers and dive professionals alike, ignore such recommendations. Everyone knows that. PADI KNOWS THAT."

So it's not just the resorts that ignore the recommendations--the entire dive industry ignores the recommendations. The message from the companies that actually run the dive boats is loud and clear: "Let's not let some legalistic mumbo jumbo mess up the dive plan (and our profit margin)".
